Services

Date Healthcare Services LLC provides these services under Medicaid CCP:

• Physical Therapy
• Occupational Therapy
• Speech Therapy
• Durable Medical Equipment Coordination

These therapies will be provided to children who have been diagnosed with medical conditions such as:

• Autism Spectrum Disorder
• Cerebral Palsy
• Developmental Delay
• Processing Disorder Resulting from ADD/ADHD
• Down Syndrome
• Traumatic Brain Injury
• Speech / Language Delay

Eligibility Requirements

• Medicaid Eligible in Texas
• Children ages 0-20 years old
• Residing in Texas
• US Citizen or Permanent Resident
• Services require a physician’s prescription
• Prior authorization of services is required
• Homebound status is not required
